The Charter is the Continuum of Care's governing document--essentially the by-laws under which this collaborative effort operates. The Charter will be updated at a minimum annually; the CoC Board may append policies or make other changes at any of its monthly meetings.


Sonoma County Continuum of Care Notice of Priority for Persons Experiencing Chronic Homelessness and Other Vulnerable Homeless Persons in Peramenent Supportive Housin

On October 27th, 2015, the CoC Board formally adopted Notice CPD-14-012 from HUD, “Notice on Prioritizing Persons Experiencing
Chronic Homelessness and Other Vulnerable Homeless Persons in Permanent upportive Housing and Recordkeeping Requirements for Documenting Chronic HomelesStatus” for all CoC funded projects. The following guidelines will be utilized for all CoC funded projects in
these revised standards in a manner consistent with each project’s current grant agreements.
