A "Continuum of Care" is:
  • A funding stream from the federal Department of Housing and Urban Development (HUD), dedicated to services and housing to end homelessness; and
  • The local planning process required in order to access those funds.

The Continuum of Care brings about $2.9 million annually to Sonoma County to end homelessness!

Sonoma County’s 10-Year Homeless Action Plan, adopted in 2007, is our strategic plan to end homelessness. This plan includes strategies to address the needs of all homeless subpopulations, and ensures that financial and other resources are coordinated to provide the necessary breadth of housing and service options, so that people experiencing homelessness are able to transition from the streets to stable work and living environments, and become more self-sufficient.

In 2015, the Sonoma County Community Development Commission released the Building Homes Toolbox. This plan details the housing needed for the success of the 10 Year Plan.
