What is "Continuum of Care?"

  • A funding stream from the federal Department of Housing and Urban Development (HUD), dedicated to ending homelessness.
  • The local Sonoma County planning process required to access these funds.
  • The Continuum of Care brings approximately $2.9 million annually to Sonoma County to end homelessness!

2017 Point in Time Homeless Census

The 2017 Point in Time Homeless Census took place on January 29, 2017 throughout Sonoma County. The full report can be found here:

2,835 people were counted during the homeless count, of which 1,847 are living in unsheltered conditions. The Continuum of Care (CoC) brings together government, service providers, and community activists to develop collaborative strategies to reduce homelessness and address the needs of many varied homeless sub-populations.

2016 Point in Time Homeless Census

Sonoma County’s 10-Year Homeless Action Plan, adopted in 2007, is our strategic plan to end homelessness. This plan includes strategies to address the needs of all homeless subpopulations, and ensures that financial and other resources are coordinated to provide the necessary breadth of housing and service options, so that people experiencing homelessness are able to transition from the streets to stable work and living environments, and become more self-sufficient.

In 2015, the Sonoma County Community Development Commission released the Building Homes Toolbox. This plan details the housing needed for the success of the 10 Year Plan.
